ABSTRACT

En esta editorial, la autora aborda la problemática de las prácticas de bajo valor en la medicina contemporánea: aquellas intervenciones terapéuticas o diagnósticas carentes de respaldo científico, que aumentan la probabilidad de daños, generan desperdicio de recursos y amenazan la eficiencia del sistema de salud. En un contexto de preocupación global por el exceso médico y las consecuencias del sobreuso de intervenciones inefectivas, resalta la relevancia del concepto de prevención cuaternaria en la atención sanitaria, y señala la iniciativa internacional Choosing Wisely como una estrategia para identificar y revertir las prácticas de bajo valor, destacando la importancia del cambio cultural y la participación activade los pacientes. Finalmente, la autora presenta el lanzamiento de Choosing Wisely Argentina, una colaboración entre asociaciones científicas locales con el compromiso de transformar la práctica médica en este país, priorizando el bienestar del paciente y adoptando un enfoque integral hacia la atención sanitaria. In this editorial, the author addresses the problem of low-value practices in contemporary medicine: those therapeutic or diagnostic interventions that lack scientific support and increase the probability of damage, generate waste of resources,and threaten the efficiency of the health system. In a context of global concern about medical excess and the consequences of the overuse of ineffective interventions, she highlights the relevance of the concept of quaternary prevention in healthcare, and points to the international Choosing Wisely initiative as a strategy to identify and reverse low-value practices, highlighting the importance of cultural change and active patient participation. Finally, the author presents the launch of Choosing Wisely Argentina, a collaboration amongst local scientific associations with the commitment to transform medical practice in this country, prioritizing patient well-being and adopting a comprehensive approach to health care. ABSTRACT

Student protagonism is paramount in the knowledge construction process. In this paper, we discuss a didactic-pedagogical resource called licentîa hypomnema (LH), inspired in portfolios and learning diaries in which student-teachers record their understandings and reflections regarding pedagogical topics learned at University in a meta-learning process on learning about teaching. The initial context of the research was a Psychological Development and Teaching course, offered in the Teacher Education program at Universidade de Brasília (UnB). Two of the authors, then Literacy and Languages Teaching students, produced their LH and peer assessed each other during the course. In their annotations, the student-authors recorded two pedagogical situations related to the use of LH in their own teaching practices, causing an impact on their formative path. These materials consist of the data for analysis and discussion. Moreover, we discuss possible uses of reflexive writing in teacher education and other learning contexts. Producing a LH supports knowledge production and allows students to connect theory and practice, which consequently influences the student's teaching practice. Since LH is a didactic-pedagogical resource and not a close-ended tool, both the context and individuals who use it should be considered.(AU)


O protagonismo de estudantes tem grande importância no processo de construção do conhecimento. Neste artigo, discutimos um recurso didático-pedagógico chamado licentîa hypomnema (LH), inspirado em portfólios e diários de aprendizagem, isto é, estudantes de licenciatura escrevem suas compreensões e reflexões acerca de assuntos pedagógicos vistos na universidade em um processo de meta-aprendizagem sobre aprender a ensinar. O contexto inicial da pesquisa foi um curso de Desenvolvimento Psicológico e Ensino, ministrado em cursos de Licenciatura da Universidade de Brasília (UnB). Duas das autoras eram estudantes do curso de Letras, elas produziram seus LH, realizando avaliação por pares entre si durante o curso. As estudantes-autoras trouxeram em seus registros duas situações pedagógicas relacionadas ao uso desse recurso em suas próprias práticas de ensino que causaram um impacto em suas trajetórias formativas. São esses relatos que compõem os dados para análise e discussão. Além disso, procuramos discutir possíveis desdobramentos para o uso do recurso da escrita reflexiva na formação docente e em outros contextos de aprendizagem. A produção do LH fornece suporte para a produção de conhecimento e permite ao aluno conectar teoria e prática, o que, consequentemente, reverbera na prática de ensino do aluno. O LH é um recurso didático-pedagógico e não uma ferramenta fechada, portanto, tanto o contexto quanto as pessoas que o utilizam devem ser considerados.(AU)


El protagonismo de los estudiantes tiene gran importancia en el proceso de construcción del conocimiento. En este trabajo, discutimos un recurso didáctico-pedagógico llamado licentîa hypomnema (LH), que se inspira en diarios de aprendizaje, es decir, estudiantes de profesorado escriben sus comprensiones y reflexiones sobre temas pedagógicos aprendidos en la universidad, produciendo un proceso de meta-aprendizaje sobre aprender a enseñar. El contexto inicial de la investigación fue un curso de Desarrollo Psicológico y Enseñanza, impartido en el programa de Formación de Profesores de la Universidade de Brasília (UnB). Dos de las autoras eran alumnas de la carrera de Letras que produjeron sus LH y realizaron una evaluación por pares mutua durante el curso. Las estudiantes autoras plantearon en su LH dos situaciones pedagógicas relacionadas con el uso de este recurso en sus propias prácticas de enseñanza, causando un impacto en su trayectoria formativa. Estos materiales constituyen los datos para el análisis y la discusión en esta investigación. Buscamos discutir posibles desdoblamientos para el uso del recurso de la escritura reflexiva en la formación docente y en otros contextos de aprendizaje. La producción de este recurso da soporte a la producción de conocimiento y permite al estudiante conectar teoría y práctica, lo que consecuentemente repercute en su práctica docente. El LH es un recurso didáctico-pedagógico y no una herramienta cerrada, por lo tanto, se debe considerar tanto el contexto como las personas que lo utilizan.(AU)

ABSTRACT

Este artigo pretende compreender as concepções de profissionais da gestão e dos serviços do Sistema Único de Saúde (SUS) sobre Educação Permanente em Saúde (EPS), bem como seus desafios e potencialidades. Utilizou-se de grupo focal para coleta, seguido de análise lexical do tipo classificação hierárquica descendente com auxílio do software Iramuteq. Os resultados delinearam quatro classes: a) EPS - entendimentos e expectativas; b) entraves à EPS; c) ETSUS e EPS por meio de cursos e capacitações; e d) dispositivos de EPS: potencialidades e desafios. Os participantes apontaram equívocos de entendimentos acerca da EPS ao equipará-la à Educação Continuada (EC) voltada à transferência de conteúdo, com repercussões negativas na prática de EPS. Discute-se o risco em centralizar o responsável pela concretização dessa proposta, que deveria ser coletiva e compartilhada entre diferentes atores. Reivindica-se, portanto, uma produção colaborativa, que possa circular entre os envolvidos, de modo que cada um experimente esse lugar e se aproprie da complexidade de interações propiciadas pela Educação Permanente em Saúde.(AU)


This article aims to understand the conceptions of professionals from the management and services of the Unified Health System (SUS) on Permanent Education in Health (EPS), as well as its challenges and potential. A focus group was used for data collection, followed by a lexical analysis of the descending hierarchical classification type using the Iramuteq software. The results delineated four classes: a) EPS - understandings and expectations; b) obstacles to EPS; c) ETSUS and EPS by courses and training; and d) EPS devices: potentialities and challenges. Participants pointed out misunderstandings about EPS, when equating it with Continuing Education (CE) focused on content transfer, with negative repercussions on EPS practice. The risk of centralizing the person responsible for implementing this proposal, which should be collective and shared among different actors, is discussed. Therefore, a collaborative production is claimed for, which can circulate among those involved, so that each one experiences this place and appropriates the complexity of interactions provided by Permanent Education in Health.(AU)


Este artículo tiene por objetivo comprender las concepciones de los profesionales de la gestión y servicios del Sistema Único de Salud (SUS) sobre Educación Continua en Salud (EPS), así como sus desafíos y potencialidades. Se utilizó un grupo focal para la recolección de datos, seguido por un análisis léxico del tipo clasificación jerárquica descendente con la ayuda del software Iramuteq. Los resultados delinearon cuatro clases: a) EPS: entendimientos y expectativas, b) Barreras para EPS, c) ETSUS y EPS a través de cursos y capacitación, y d) Dispositivos EPS: potencialidades y desafíos. Los participantes informaron que existen malentendidos sobre EPS al equipararla a Educación Continua, con repercusiones negativas en la práctica de EPS, orientada a la transferencia de contenidos. Se discute el riesgo de elegir a un solo organismo como responsable de implementar esta propuesta colectiva, que debería ser colectiva y compartida entre los diferentes actores. Se aboga por un liderazgo colaborativo, que pueda circular entre los involucrados, para que cada uno experimente este lugar y se apropie de la complejidad de interacciones que brinda la Educación Continua en Salud.(AU)

Objetivo: Monitorar o alcance das metas pactuadas nos planos de enfrentamento das Doenças Crônicas Não Transmissíveis (DCNTs). Métodos: Estudo transversal, com dados da Pesquisa Nacional de Saúde (PNS) de 2013 e 2019. Avaliaram-se as metas, até 2025, para inatividade física, consumo de bebidas alcoólicas, sal/sódio, uso do tabaco, hipertensão arterial, diabetes, excesso de peso, obesidade, cobertura do Papanicolau e terapia medicamentosa e aconselhamento pelo cálculo de razões de prevalência (RP). Resultados: Foram avaliados 60.202 indivíduos em 2013 e 88.531 em 2019. As metas para inatividade física (RP = 0,88; IC95% 0,86;0,90) e cobertura do Papanicolau (79,4%; IC95% 78,3;80,3) foram alcançadas. Reduziu-se o uso do tabaco, mas abaixo da meta. As prevalências de hipertensão, diabetes, excesso de peso, obesidade e consumo de bebidas alcoólicas aumentaram, e as metas não serão atingidas. Conclusão: Dois indicadores alcançaram as metas pactuadas, contudo é necessário avançar em ações e políticas para cumprir as demais.

Métodos: estudio transversal, con datos de la Pesquisa Nacional de Saúde 2013 y 2019. Se evaluaron las metas hasta 2025: inactividad física, consumo de alcohol, sal/sodio, tabaquismo, hipertensión, diabetes, sobrepeso, obesidad, Papanicolaou cobertura y terapia y asesoramiento farmacológico. Calculado la razón de prevalencia (RP). Resultados: Un total de 60,202 individuos fueron evaluados en 2013 y 88,531 en 2019. Se alcanzaron los objetivos de inactividad física (RP = 0,88; IC95%: 0,86;0,90) y cobertura de la prueba de Papanicolaou (79,4%; IC95%: 78,3;80,3). El consumo de tabaco se redujo, pero por debajo del objetivo. Se incrementó hipertensión, diabetes, sobrepeso, obesidad y consumo de alcohol y no se alcanzarán las metas. Conclusión: Dos indicadores alcanzaron las metas acordadas, sin embargo, es necesario avanzar en acciones y políticas para cumplir con los demás.


Objective: To monitor the achievement of the action plans for the prevention and control of Non-Communicable Diseases agreed-upon targets. Methods: Cross-sectional study, with data from the 2013 and 2019 National Health Survey. The following targets, up to 2025, were evaluated: physical inactivity, alcohol consumption, salt/sodium, tobacco use, high blood pressure, diabetes, overweight, obesity, cervical cytology testing, and drug therapy and counseling. To check whether the targets were achieved, the prevalence ratio was calculated (PR). Results: 60,202 individuals were assessed in 2013, and 88,531 in 2019. The targets for physical inactivity (PR = 0.88; 95%CI 0.86;0.90) and cervical cytology coverage (79.4%; 95%CI 78.3;80.3) were achieved. Tobacco use was reduced, albeit below the target. The prevalence of hypertension, diabetes, overweight, obesity and alcohol consumption increased, and the targets will not be attained. Conclusion: Two indicators reached the agreed targets, however it is necessary to advance in actions and policies to meet the others.

ABSTRACT

Nuestra institución universitaria cuenta con tres funciones sustantivas que son docencia, proyección social e investigación. Desde el año pasado se vienen alineando las dependencias para corresponder con cada una de las mismas, siendo este el motivo por el que la Fundación Universitaria de Ciencias de la Salud generó una modificación importante en su organigrama al gestar dos nuevas vicerrectorías, que acompañan a la reconocida vicerrectoría académica, y lideran las funciones sustantivas de proyección social e investigación, siendo subordinadas de nuestra Rectoría con el fin de alcanzar las metas institucionales planeadas. En medio de esos cambios es para mí un honor haber sido designado desde septiembre del año anterior por el Consejo Superior como el nuevo Vicerrector de Proyección Social, cargo en el que tendré la labor de dirigir áreas que son reconocidas por el cumplimiento de sus objetivos y por sus interacciones con todas las dependencias de la universidad, así como con diversas entidades en el entorno local, distrital, nacional e internacional.

ABSTRACT

Induction of labour (IOL) is defined as an artificial stimulation administered to initiate the delivery process before the onset of spontaneous labour. Setting-adapted guidelines need to be developed to promote safe maternal and neonatal care in line with the needs of a specific institution. This study aimed to describe and assess the current IOL practices at Botshabelo District Hospital, focusing on incidence, indications, induction methods, complications, and outcomes. A retrospective-descriptive study included all relevant data from IOL cases over six months between July and December 2017. From 168 attempted inductions of labour, 153 files were retrieved. The majority of cases (69.7%) were for post-dates. Normal vaginal delivery (NVD) was achieved in most patients (69.3%), while one patient had an assisted delivery. Thus, 30.1% of inductions failed and required caesarean sections. The incidence, indications, methods of induction, complications, and outcomes of IOL in BDH are in line with international guidelines; however, including the sweeping of membranes at term and balloon catheters as methods could improve the current guidelines.

Objective: This study aimed to assess the objective understanding of five front-of-pack label formats, namely the Reference Intake, Multiple Traffic Light label, Nutri-Score, health endorsement logo and warning label, among consumers in Nelson Mandela Bay, South Africa. Design: This was a cross-sectional, exploratory study. Setting: Interviewer-administered surveys were conducted at 12 randomly selected food retail outlets in an urban setting.Subjects: A total of 359 adult participants were included in this study. Outcome measures: Objective understanding was measured by asking participants to rank food products according to their nutritional quality, using the displayed front-of-pack label. Results: The type of front-of-pack label significantly influenced ranking ability (p < 0.00001). All the evaluative front-of-pack labels significantly improved the participants' ability to identify healthier food products compared with the no-label control. The reductive Reference Intake, however, showed no significant impact on consumer understanding. Conclusion: Evaluative front-of-pack labels significantly improved the participants' ability to identify healthier food products, when compared with the Reference Intake and no-label control. Future research should test the objective understanding of culturally diverse groups in South Africa, particularly among rural populations.

ABSTRACT

Este artigo analisa como a instrumentalização do coaching pelas organizações, enquanto prática gerencial, propicia a reprodução de preceitos gerencialistas que se alinham ao sequestro da subjetividade do trabalhador. Para tal, efetuaram-se entrevistas semiestruturadas com 11 gestores que conduziam tal intervenção no ambiente de trabalho. A interpretação dos dados fundamenta-se na Análise Crítica do Discurso, textualmente orientada. Embora o coaching no trabalho suscite reflexividade, tal prática encontra-se a serviço do ideal gerencialista, reafirmando o ideário social de culto ao desempenho, que propaga auto (e alta) responsabilização individual. O paradoxo é que a subjetividade - hiper solicitada para dar lugar a uma "subjetividade realizadora" - foi representada discursivamente como entrave ao desenvolvimento pessoal, o qual é indissociado de anseios organizacionais. Por fim, discute-se o papel da resistência no bojo do referido processo de intervenção.


This article analyzes how the instrumentalization of coaching by organizations, as a managerial practice, provides the reproduction of managerial precepts that are in line with the kidnapping of the worker's subjectivity. To this end, semi-structured interviews were carried out with 11 managers who conducted such intervention in the workplace. Data interpretation is based on the textual oriented Critical Discourse Analysis. Although coaching at work raises reflexivity, such pratice is at the service of the managerial ideal, reaffirming the social ideal of performance worship, which propagates individual self (and high) responsibility. The paradox is that subjectivity - hyper-requested to give way to a "fulfilling subjectivity" - was represented discursively as an obstacle to personal development, which is inseparable from organizational concerns. Finally, the role of resistance within the aforementioned intervention is discussed.

ABSTRACT

El propósito de este estudio consistió en revisar el modelo del Balanced Scorecard y analizar su aplicabilidad como instrumento de gestión en los centros de salud. El tipo de investigación fue de tipo documental, con un diseño no experimental. Para alcanzar el objetivo, se revisaron los postulados teóricos de Kaplan y Norton, así como algunas implementaciones prácticas centradas en este paradigma que fueron desarrolladas en Norteamérica, Europa y Sur América. Los resultados demuestran el alto valor agregado que ofrece el Balanced Scorecard como instrumento de gestión en centros de salud.


The purpose of this study was to review the Balanced Scorecard model and analyze its applicability as a management instrument in health centers. The type of research was documentary, with a non-experimental design. To achieve the objective, the theoretical postulates of Kaplan and Norton were reviewed, as well as some practical implementations focused on this paradigm that were developed in North America, Europe and South America. The results demonstrate the high added value that the Balanced Scorecard offers as a management instrument in health centers.

ABSTRACT

ABSTRACT Objective: To discuss the political and structural conditions for establishing the Unified Health System (UHS - Sistema Único de Saúde, SUS) in coping with the SARS-CoV-2 pandemic. Methods: Theoretical-reflection study. Results: At the first moment named "The global and the local in facing the SARS-CoV-2 pandemic" is presented the health crisis that took place worldwide and the government actions to combat COVID-19. A second moment named "Between dismantling actions and resistance, the UHS is the best way to face the SARS-CoV-2 pandemic", reflects on the neoliberal attacks on the health system and how it resists, remaining the main pandemic response strategy. Conclusion: The strengthening of democracy and the defense of the UHS are the way out of the crisis. It is believed that this reflection generates - in everyone who deals with caretaking - the political action, the ethical attitude, the desire for valorization and the spirit of struggle in defense of the UHS and human life.


RESUMO Objetivo: Discutir las condiciones política estructurales de efectuación del SUS en el enfrentamiento de la pandemia por SRAS-CoV-2. Métodos: Estudio teórico-reflexivo. Resultados: En el primer momento, intitulado, "El global y el local en el enfrentamiento de la pandemia por SRAS-CoV-2", se presenta la crisis sanitaria que se ha instalado en el mundo y las acciones gubernamentales para el combate a la COVID-19. En un segundo momento, intitulado, "Entre acciones de desmonte y de resistencia, el SUS es el mejor camino para el enfrentamiento de la pandemia por SRAS-CoV-2", reflexionarse sobre los ataques neoliberales al sistema y como esto resiste, siendo la principal estrategia de respuesta a la pandemia. Conclusión: El fortalecimiento de la democracia y la defensa del SUS son la salida para el enfrentamiento de la crisis. Se cree que esta reflexión genere — en todos que lidian con el cuidado — el actuar político, la actitud ética, el deseo de valorización y espirito de lucha en defensa del SUS y de la vida humana.


RESUMO Objetivo: Discutir as condições político-estruturais de efetivação do SUS no enfrentamento da pandemia por SARS-CoV-2. Métodos: Estudo teórico-reflexivo. Resultados: No primeiro momento, intitulado, "O global e o local no enfrentamento da pandemia por SARS-CoV-2", apresenta-se a crise sanitária que se instalou no mundo e as ações governamentais para o combate à COVID-19. Em um segundo momento, intitulado, "Entre ações de desmonte e de resistência, o SUS é o melhor caminho para o enfrentamento da pandemia por SARS-CoV-2", reflete-se sobre os ataques neoliberais ao sistema e como este resiste, sendo a principal estratégia de resposta à pandemia. Conclusão: O fortalecimento da democracia e a defesa do SUS são a saída para o enfrentamento da crise. Acredita-se que esta reflexão gere — em todos que lidam com o cuidado — o agir político, a atitude ética, o desejo de valorização e espírito de luta em defesa do SUS e da vida humana.
